New Patient Forms
We are always welcoming new patients to our Arlington dental practice. We look forward to your visit! Before you come, please download our new patient forms, print them out, and bring the completed forms to our office to expedite the paperwork at your first visit for your convenience. If you would prefer to fill out your new patient paperwork at our office, please arrive 10-15 minutes before your scheduled appointment so there will be sufficient time.

For those who are joining our family of patients, it might seem out of the ordinary to be scheduled for a 90-minute new patient visit with one of our dentists and a hygienist. This time is reserved especially for you to ensure that you receive a thorough examination and consultation. Our goal is not only to take care of our patients' dental needs, but also to get to know each person by name and treat them like we would want our own family to be treated.
Scheduling
We know that fitting quality dental care into your busy life isn't easy. We offer convenient hours, including evening and Friday dental appointments, and we strive to run on time as we understand how valuable your time is. Our doctors only deviate from our schedule in the event of dental emergencies that need to be taken care of immediately. In return, we ask patients to arrive on time — if we all work together, things run more smoothly and we can maximize patient convenience.
Once we have decided on a treatment plan and have worked out financial arrangements, we will do everything we can to proceed with your dental treatment on a schedule that works for you, whether that be immediate treatment or at a slower pace.
Missed Appointment and Cancellations
Please call at least two business days in advance for changed appointments. Missed appointments without this notification, or repeated cancellations, will incur cancellation fees. Contact us with any questions or to schedule your appointment. We look forward to seeing you soon!
